How to Use Evaluation Forms for Structured Interviews

Use evaluation forms to collect consistent, structured feedback from interviewers and make better hiring decisions.


What are evaluation forms


Evaluation forms are structured questionnaires interviewers complete after interviews. They ensure:


  • Consistent criteria across candidates
  • Standardized feedback format
  • Comparable interview data
  • Better hiring decisions


Creating an evaluation form


  1. Go to Settings / Evaluation Forms
  2. Click Add form
  3. Add questions:
  • Rating scales (1-5, 1-10)
  • Multiple choice
  • Text fields for comments
  • Yes/No questions
  1. Name the form
  2. Save


Example questions


Technical interview form

  • "Rate technical problem-solving ability (1-5)"
  • "Rate code quality and best practices (1-5)"
  • "What technical strengths did you observe?"
  • "What concerns do you have?"
  • "Recommend for hire? (Yes/No/Maybe)"


Culture interview form

  • "Rate communication skills (1-5)"
  • "Rate cultural fit (1-5)"
  • "How well do they align with company values?"
  • "Would you enjoy working with this person?"
  • "Overall recommendation"


Assigning forms to jobs


  1. Open a job
  2. Go to any candidate's profile
  3. Go to Evaluation tab
  4. Click Assign Evaluation Forms
  5. Select forms to use for this job


Using during interviews


Before the interview

  • Interviewers see assigned evaluation forms
  • Can review questions in advance
  • Prepare structured interview approach


During/after the interview

  • Interviewer opens candidate profile
  • Goes to Evaluation tab
  • Fills out the form
  • Submits feedback


Completion reminders


100Hires sends daily reminder emails to interviewers until they complete evaluations:


  1. After an interview is scheduled
  2. Daily reminders continue until form is submitted
  3. Configure at Settings / Notifications


Viewing evaluation results


  1. Open candidate profile
  2. Go to Evaluation tab
  3. See all submitted evaluations
  4. Compare feedback across interviewers


Blind evaluations


Enable blind evaluations to reduce bias:


  1. Go to Settings / Company
  2. Enable Blind evaluations
  3. Submit your own feedback before viewing feedback from other interviewers


Admins can always view all evaluations.


Tips


  • Keep forms focused (5-10 questions)
  • Use consistent rating scales
  • Include both ratings and open-ended questions
  • Require key questions to prevent incomplete submissions
  • Review forms periodically and update based on hiring needs
