How to Assess Candidates with Questionnaires
Use built-in questionnaires instead of external tools like Typeform to collect candidate information while keeping data in 100Hires for automation and AI scoring.
Where to Find Questionnaires
All forms are created in Settings / Forms. A form becomes a Questionnaire when you send it via email template link (rather than using it on a job's Application tab).

Why Use Built-in Questionnaires
- Data stays in 100Hires: No need to export/import
- Automation triggers: Act when forms are filled
- AI scoring: Score candidates based on responses
- Unique links: Each candidate gets their own link
- Response tracking: See who completed and who didn't
Creating a Questionnaire
- Go to Settings / Forms
- Click Add form

- Add questions:
- Text questions for short answers
- Textarea for long answers
- Dropdowns for multiple choice
- Checkboxes for yes/no
- Number fields for numeric data
- Name and save the questionnaire

Sending Questionnaires
In Email Templates
- Go to Settings / Email Templates
- Create or edit a template
- Click Questionnaires button

- Select your questionnaire
- Placeholder is inserted

- Each candidate receives unique link
In Nurture Campaigns
- Create a nurture campaign
- Add email step with questionnaire template

- Candidates receive questionnaire automatically
Automating Based on Responses
Move to Stage When Completed
- Go to job's Workflow tab
- Add Act If Form Filled automation
- Condition: Required question is not empty

- Action: Move to next stage
Disqualify Based on Answer
- Add Act If Form Filled automation
- Condition: [Question] contains "No" (or specific answer)
- Action: Disqualify

AI Score Using Responses
- Set up AI Scoring criteria that reference questionnaire answers

- Add AI Score automation after questionnaire completion

- AI analyzes responses and scores candidates
Viewing Responses
- Open candidate's profile
- Go to Forms tab (left panel)
- See all questionnaire responses

- Responses also appear in Discussion tab
